In the present paper a methodology for the fine control of modern linear accelerator generated MV photon beam to provide adequate dose distribution on the irregular surface of a superficial region on the patient body using indigenously design bolus has been reported. In this report a radiotherapy case study of a patient in which a right eye lid tumor infiltrating towards outer canthus area (orbital plasamacytoma) was treated with modern radiotherapy technique (intensity modulated radiotherapy) by using wet- cotton bolus . The bolus gives adequate dose coverage on the superficial tumor area. The plan was designed by pinnacle radiotherapy treatment planning system and measured the dose by using perpex phantom with ionization chamber and also by film dosimeter. The result obtained is found to be good agreement with the recommended value with a deviation limit of +/- 5%. The patient was treated three years back in the research centre, now it is found that the patient is living clinically disease free survival with regular follow up. Though the particular case addressed belongs to the orbital plasmacytoma, the technique is also useful for the treatment of other sites as well. Hence, the present work is interesting and useful for the medical physics community in general and radiation-health physicists in particular.
